DOULA SERVICES
AGREEMENT 

  1. Nature of Doula Services


The Doula provides non-medical emotional, informational, and physical support during pregnancy, labor birth, postpartum, and pregnancy loss when requested.

The Doula does not:


  • Perform medical procedures

  • Provide clinical care

  • Diagnose conditions

  • Make medical decisions for the client


Clients agree to maintain care with their chosen licensed medical provider and to seek appropriate medical care throughout pregnancy, labor, birth, and postpartum as needed.



  1. Scope of Services


The Doula provides non-medical support services as outlined in the client's selected package. Specific services, visits, and support included in the client's care are determined by the package selected and described in this agreement. Services may include:


  • Prenatal visits

  • Labor and birth support

  • Comfort measures

  • Birth Preference Planning

  • Emotional and Trauma-informed support

  • Partner support

  • Postpartum visits

  • Bereavement support when applicable


Exact services are determined by the selected package.

  1. On Call Period


The Doula will be on call beginning at 37 weeks of pregnancy and continuing through the birth, unless otherwise stated in this agreement.


The client agrees to notify the Doula as soon as reasonably possible when labor begins, when traveling to the hospital or bith center, and upon admission to the hospital or birth center. Failure to notify the Doula in a timely manner may limit the Doula's ability to provide continuous labor and birth support and may effect availability.until birth unless otherwise stated.


  1. Length of Labor Support


The Doula provides continuous labor and birth support once active labor has begun.

If labor extends beyond approximately 18–24 hours, the Doula may take reasonable breaks for rest or food, hydration, and personal needs, while maintaining appropriate communication and support.


If necessary for the safety and effectiveness of care, the Doula may arrange for a qualified backup doula to provide temporary or continued support. The Doula will communicate with the client regarding any changes in support whenever reasonably possible.

 

  1. Back Up Doula


In the event of illness, family emergency, overlapping births, unsafe conditions, or other circumstances that prevent the Doula from attending the birth, a qualified backup doula may provide support. The client will be noticed as soon as reasonably possible, and the backup doula's names are provided below and contact information will be provided when applicable.

  1. Fees & Payment


The total package fee, retainer amount, and remaining balance are outlined in the invoice provided to the client once Doula Services agreement is signed and received.


By signing this agreement, the client acknowledged and agrees to the full payment amount as stated in the invoice.


Final payment must be completed by the due date listed on the invoice. Failure to complete payment may result in suspension of services.


Payment schedule: The remaining balance may be paid in installments at the clients convenience prior to the birth. All payments must be completed in full no later than agreed due date. No balance may remain after birth.


The retainer is non-refundable and secures the client’s estimated due date in the doula’s calendar.

Failure to pay the balance by the due date may result in suspension of services.


Travel Fees:


Our service area includes Middle Georgia and surrounding regions. Travel outside of 45 miles from Perry, Ga may include a travel fee.

45 - 75 miles -$75


75 - 120 miles - $150


  1. Cancellation Policy


If the client cancels services:


  • Retainer is non-refundable

  • Any refund of amounts paid beyond the non-refundable retainer will be determined based on services already provided and any applicable amounts owed under under this agreement.

Additional terms:


  • If the client hires another doula or declines services after booking, the retainer remains non-refundable.

  • Should the mother or partner fail to notify the doula immediatelyupon being aware of the client being in labor, and the doula misses the birth, of no fault of her own doing, the full fee will be retained and balance must be paid.

  • If mother goes into labor prior to full payment being made, the full payment is still required to be paid.

  1. Early Birth or Rapid Labor


If the client experiences an early or rapid labor and the Doula is unable to arrive before the birth due to rapid labor, late notification, or other unexpected circumstances beyond the Doula's reasonable control, this does not constitute a failure to provide services.


Postpartum support will still be provided according to the package, as applicable.


The client agrees to notify the Doula as soon as possible when labor begins or when they believe birth may be imminent.

  1. Cesarean Birth


If a cesarean birth occurs, the Doula will provide emotional, informational, and physical support before and after surgery when permitted by the hospital's policies and the medical team.

The Doula may not be permitted in the operating room or may be required to remain outside the operating room, depending on hospital policy, facility restrictions,and the circumstances of the birth.


If the Doula is unable to enter the operating room, support will continue before surgery and resume as soon as reasonably possible following the procedure, when permitted.

  1. Pregnancy Loss, Change in Birth Outcome, or Bereavement


If miscarriage, stillbirth, infant loss, or other pregnancy or birth loss occurs, the Doula will offer compassionate, emotional support and will discuss with the client whether existing services should be continued, adjusted, or concluded based on the circumstances.


Bereavement support offered by Oh! BabyBirth Companion & Beyond is not limited to pregnancy or birth related loss and may include support following the loss of a loved one or other significant loss of life.


If additional bereavement support is desired and available, the Doula will discuss the appropriate services and any applicable fees with the client before those additional services are provided.

  1. Health & Safety


The Doula may decline or temporarily suspend in-person services when conditions present a reasonable risk to the health or safety of the Doula or client. This may include contagious illness, contagious illness, extreme weather conditions, or other circumstances that may reasonably create a health or safety concern.


When reasonably possible, alternative support such as phone, text or virtual support will be offered. In person support will resume when conditions are reasonably safe and appropriate.

  1. Confidentiality


All client information shared with or obtained by the Doula in connection with services will be treated as confidential and will not be disclosed to others without the client's written consent, except as required or permitted by law.


The Doula will make reasonable efforts to protect the privacy of client information and communications.

  1. Media Consent (Optional)

The client may choose whether to give Oh! BabyBirth Companion & Beyond permission to use photographs, stories, testimonials, or other media for educational or promotional purposes.

Media consent is voluntary and is not required to recieve doula services.

If niether permission is selected, no media, stories, testimonials, or photographs will be used.


The client may withdraw permission for future use by providing written notice to the Doula.

  1. Limitation of Liability


The client acknowledges that the doula provides non-medical support services and does not provide medical care, diagnosis, treatment, or medical advice, The Doula is not responsible for medical outcomes, complications, or decisions made by the client or by medical providers. The client remains responsible for their own healthcare decisions and agrees to seek medical care and follow the recommendations of their qualified healthcare providers when appropriate.

The doula is not responsible for medical outcomes or decisions made by medical providers or the client.

  1. Agreement and Signatures


By signing below, both parties acknowledge that they have read, understood, and agreed to the terms and conditions of this contact. The signatures below indicate acceptance of this agreement and the responsibilities outlined herein.
